• Chemical Vulnerability (The Garden Grove Leak): The emergency evacuation involving GKN Aerospace was triggered by a mechanical valve failure on a tank holding methyl methacrylate. The chemical experienced "thermal runaway," a vicious cycle where a chemical reaction produces heat, and that heat further accelerates the reaction, causing the tank to bulge and threaten to explode. 
• Refinery System Strain: Major fires at facilities like the Chevron El Segundo Refinery or Martinez Refining Company are complex accidents. Independent safety reports often cite structural wear, poor training, and inadequate supervision during routine maintenance rather than external interference.
